Social Prescribing is a holistic approach to healthcare that combines social and medical models of health and wellness. It bridges the gap between clinical and social care by referring patients to local, non-clinical services chosen according to the client’s interests, goals, and gifts. These community-led, non-clinical services remove barriers to health equity while empowering clients to better manage their own health, develop new skills, and build connections with people who have similar interests and backgrounds.
